Fax completed forms to 955-0293 Attn: Ben Gleaton or hand-delivered to 711 E. Porter Street (East of Fremont Teed Elementary). Please include a copy of your organizations certificate of liability insurance listing Kuna Jt. School District No. 3 as a certificate holder.
