If you are interested in participating in a sport at KHS for 2012-2013 year, please print off the following forms, sign and submit to the main office. Mandated Paperwork List - all must be turned in before an athlete can practice KHS will offer SPORTS PHYSICALS on May 20 & 21 from 6-8 pm Concussion Parent Acknowledgement Form
